SHOT RECORDS MUST BE CURRENT BY TEXAS STATE REQUIREMENTS TO ENROLL
Kindergarten Entry- If your child is entering kindergarten they must be current by kindergarten requirements which requires shots on or after their 4th birthday.(see chart below). Hep B shots that were given in the hospital at birth will need a date for the record to be current, this is a series of 3 shots. Records from daycare might not be current for entry into Kindergarten. STATE IMMUNIZATION REQUIREMENTS
Pre K - 12TH
Students attending Wylie ISD must meet Texas immunization requirements. If you have questions concerning your child's immunization record please contact the school nurse, your physician or the Texas Dept. of Health at 325-692-5600. Current students may also view and print your child's immunization record by logging on to family access.
- Diptheria-Tetanus-Pertussis (DTP/DTaP/DT/Td/TdaP)
Minimum of four (4) doses for ages 15 months through four (4) years.
Minimum of four (4) doses for ages five (5) and six (6) years with at least one dose on or after their 4th birthday.
Minimum of three (3) doses for ages seven (7) years and older with one dose on or after their 4th birthday.
Upon entry into 7th grade, students will be required to have 1 booster dose of a tetanus/diphtheria/pertussis-containing vaccine (Tdap) if at least 5 years have passed since the last dose of tetanus-containing vaccine.
Students in grades 8-12 are required to have 1 booster dose of a tetanus/diphtheria/pertussis-containing vaccine (Tdap) 10 years after the last tetanus/diphtheria dose. TD is acceptable in place of Tdap if a medical contraindication to pertussis exists.
- Polio (IPV, OPV)
Minimum of three (3) doses with at least one (1) dose received on or after their 4th birthday. Not required for persons 18 years of age or older.
- MMR ( MEASLES, MUMPS, RUBELLA)
- The 1st dose of MMR must be received on or after the 1st birthday.
For K – 4th grade, two doses of MMR are required.
For 5th - 12th grade, two doses of a measles-containing vaccine, and one
dose each of rubella and mumps vaccine is required.
- Haemophilus Influenzae Type B (HIB)
One (1) dose of vaccine on or after 15 months of age or two (2) doses on or after 12 months of age or three (3) doses with 1 on or after 12 months through age 4. History of disease after second birthday must be verified by a signed physician's statement.
- Hepatitis B
Three (3) doses of vaccine for students in grades EC-12.
- Chicken Pox (Varicella)
Two (2) doses of vaccine on or after first birthday for students in grades KG-4 and 7-11. The two doses should be at least 28 days apart. One dose of vaccine on or after first birthday for EC, PK, grades 5-6, and grade 12. If vaccine received on or after 13th birthday, two (2) doses of vaccine are required and must be at least 28 days apart. History of disease accepted with signed physician statement or signed parent validation form.
- Hepatitis A
Two (2) doses of vaccine on or after first birthday and six (6) months apart for students in grades EC, PK, grades KG-4.
- Pneumococcal (Prevnar, PCV13)
One (1) dose of vaccine on or after second birthday, or two (2) doses of vaccine on or after first birthday, or four (4) doses of vaccine in infancy with one (1) after first birthday for students through age 4.
- Meningococcal (Menatra, MCV4)
One (1) dose for students in grades 7-11.

School and Child Immunizations are given on a walk-in basis; Mondays, Tuesdays, Thursdays and Fridays, 9:00 am to 4:00 pm and 9:00 am to 6:00 pm on the 1st Tuesday of each month. No appointment is necessary. Eligibility requirements.
